Results from the program’s first Annual Participant Satisfaction Survey show that 85% of participants reported being satisfied with their home care visits.
Home-based care plays an important role in helping older adults remain safe, independent, and supported while living in their own communities. Through the PACE model, interdisciplinary care teams coordinate services that may include nursing visits, therapy, personal care, and other support delivered directly in participants’ homes.
These services ensure that participants receive consistent, high-quality care regardless of location. The same commitment to safety, compassion, and clinical excellence found in Colorado PACE centers extends to home visits throughout the community.
