The Enrollment Process

Enrollment into Colorado PACE includes four steps:

    Step 1: Referral and Intake

    The intake process begins when you, or someone on your behalf, contacts Colorado PACE. An Enrollment Specialist will visit with you to explain our program and obtain further information about you. During this visit:

    • – You will learn how the program works, and the kinds of services offered. You will receive answers to any questions you have about PACE.
    • – If you enroll, you must agree to receive all your health care services from Colorado PACE and our network of providers. Your primary care provider becomes a Colorado PACE primary care provider.
    • – You will sign release of information forms, which allow Colorado PACE to obtain your past medical records, allowing our team to fully assess your health conditions, as well as financial information and your eligibility status for Medicare and Medicaid.

    Step 2A: Financial Information

    The Colorado PACE Enrollment Specialist will discuss the various financial options available based on your financial situation. Options for payment include Medicare, Medicaid, or private pay.

    • – If you are eligible for Medicaid, or dually eligible for both Medicare and Medicaid, you won’t pay a monthly premium for Colorado PACE.
    • – If you don’t qualify for Medicaid, but you have Medicare, you will be charged a monthly premium for Colorado PACE.
    • – If you don’t have Medicare or Medicaid, you can pay for Colorado PACE privately.

    Step 2B: Functional Eligibility Determination

    Colorado PACE serves people 55 years of age and older that qualify for nursing home level-of-care according to the State of Colorado guidelines. An assessor must complete a functional assessment with you. The assessor will gather information on your functional status, nutrition risks, physical/mental health, and living arrangements.

    Step 3: Assessment

    Once your functional and financial eligibility is established, Colorado PACE will schedule an assessment at the Day Center. Additional assessments for enrollment may occur in the home. Members of the Colorado PACE Interdisciplinary Team (IDT) will meet with you individually to discuss your needs and goals. IDT members include the primary care provider, registered nurse, social worker, physical therapist, occupational therapist, recreational therapist, dietitian, home care coordinator, personal care attendant, transportation driver, and center director.

    Colorado PACE encourages you to include a family member, caregiver, or other representative that will be involved in your current or future care needs at the site visit and home visits. Collaboration of care with you, your IDT, and your caregiver(s) if applicable, is essential to identify your care plan goals. Following the assessment process, the IDT will determine if the program can appropriately meet your needs and if you have met all the eligibility criteria.

    Step 4: Enrollment

    The Colorado PACE representative will meet with you and if you wish, your family or caregiver(s), to review and sign the appropriate enrollment materials and answer any questions you may have. During this time, we will discuss:

    • – Any financial obligation, including any Medicaid client obligation identified by the Colorado Department of Human Services.
    • – Any other questions or concerns you have regarding the program.
    • – Coordination of your enrollment date into the Colorado PACE Program. The effective date of your enrollment will be the first day of the month following the date we receive your signed Enrollment Agreement.

How Do People Qualify For Colorado PACE?2024-06-30T22:22:30-06:00

A person qualifies if she or he:

  • Is 55 years of age or older.
  • Meets the requirement for skilled nursing home care as determined by Colorado PACE’s interdisciplinary team (IDT) assessment and certified by the Colorado Department of Human Services.
  • Lives in a service area (county and zip code) served by the Colorado PACE program.
  • Can live in the community without jeopardizing his/her health or safety.
Are People Who Do Not Qualify for Medicaid Eligible for Colorado PACE Enrollment?2024-06-30T22:22:19-06:00

Yes. If a person meets the income and assets limits to qualify for Medicaid, the program pays for a portion of the monthly Colorado PACE premium. Medicare pays for the rest. If a person does not qualify for Medicaid, he or she is responsible for the portion of the monthly premium Medicaid would pay. Colorado PACE staff can help determine a person’s Medicaid eligibility.

If I Belong to a Medicare HMO, Can I Also Enroll in Colorado PACE For Long-Term Care Services?2024-06-30T22:22:02-06:00

The Colorado PACE program is both Medicare providers and long-term care providers. By enrolling in the Colorado PACE program, you will no longer be enrolled in a traditional Medicare/HMO plan if you have one.

Can I Keep My Own Doctor?2024-09-02T10:26:10-06:00

Colorado PACE participants agree to receive all health care services defined in a participant’s individualized care plan developed by the Colorado PACE program’s Interdisciplinary Team (IDT). Colorado PACE IDT members include the primary care provider, registered nurse, social worker, physical, occupational, and recreational therapists, dietitian, home care coordinator, personal care attendant, transportation driver, and center director.

If you have a specialist that is part of the Colorado PACE network of contracted providers, you may be able to keep your current specialty care physician. When a participant enrolls in a Colorado PACE health plan, she/he is assigned a primary care physician (PCP) who is a member of the Colorado PACE Day Center’s Interdisciplinary Team (IDT) skilled in treating the healthcare needs of seniors.

Colorado PACE participants may be fully and personally liable for the costs of unauthorized or out of Colorado PACE program agreement services.

What Happens if I Choose to Disenroll in Colorado PACE?2024-09-02T10:32:55-06:00

Colorado PACE participants are free to disenroll from PACE at any time and resume their benefits in the traditional Medicare and Medicaid programs. You can enjoy PACE benefits until the end of the month in which you disenroll.
