What is Pace?
Colorado PACE is a non-profit senior healthcare organization that provides nursing home eligible seniors with the services needed to be able to live independently in their own homes and communities for as long as it is safely possible. Colorado PACE is a Health Plan with a Medicare Contract.
We are committed to helping you:

The Coloardo Pace Interdisciplinary Team (IDT)
Colorado PACE offers a comprehensive continuum of services customized for each individual participant.
The IDT team at the Colorado PACE Day Center coordinates all your health services.

Benefits of PACE
- Colorado PACE makes it easier to get the senior care services you need, all in one place, so you don’t have to deal with confusing or scattered services.
- Care is available 24 hours a day, every day of the week.
- A team of doctors, nurses, and other professionals, called the Interdisciplinary Team (IDT), will work with you and your family or caregiver to create a care plan that fits your needs.
- Your main doctor is located at the Colorado PACE Day Center, where you’ll get most of your healthcare.
- You won’t have to pay any co-pays or deductibles for medicine, care, or services that your care team approves.
- Transportation is arranged from home to the Colorado PACE Day Center for health care services approved by IDT and included in your personalized care plan.
- Continuous support for you, your caregiver and/or family is available.
- A caring team will be there for your medical, emotional, and social needs. We want you to stay as independent as possible while treating you with dignity and respect.
