Their lives are filled with lessons learned, challenges overcome, accomplishments celebrated and memories that help connect generations.&#xd;&#xa;&#xd;&#xa;National Senior Citizens Day is also a wonderful opportunity for all of us to show our appreciation. Spend meaningful time with an older family member or friend. Call someone you haven&#x27;t talked to in a while. Listen to their stories. Offer assistance when it is needed.     <description>Independence is important at every stage of life. For older adults, the ability to remain independent can mean staying connected to a familiar community, maintaining daily routines, making personal choices and continuing to live life on their own terms.&#xd;&#xa;&#xd;&#xa;At Colorado PACE, helping older adults maintain that independence is at the heart of what we do.&#xd;&#xa;&#xd;&#xa;Our recent participant satisfaction survey provides an encouraging look at the impact of our approach&#x3a; more than 50&#x25; of participants who responded to the survey reported that they are living independently.&#xd;&#xa;&#xd;&#xa;Supporting Independence Through PACE&#xd;&#xa;PACE, or Program of All-Inclusive Care for the Elderly, brings together a coordinated team of health care and supportive services designed around each participant&#x27;s individual needs.&#xd;&#xa;&#xd;&#xa;Rather than focusing on just one aspect of a person&#x27;s health or well-being, PACE takes a comprehensive approach. Participants have access to services and a team that works together to help them remain healthy, engaged and as independent as possible.&#xd;&#xa;&#xd;&#xa;For many older adults, that support can make a meaningful difference in their ability to continue living in their homes and communities.&#xd;&#xa;&#xd;&#xa;Independence Looks Different for Everyone&#xd;&#xa;Being independent does not necessarily mean doing everything alone.&#xd;&#xa;&#xd;&#xa;Sometimes, independence means having the right support to safely manage daily life. It can mean receiving health care, assistance or other services while continuing to make your own decisions and remain connected to the people and places that matter most.&#xd;&#xa;&#xd;&#xa;That is one of the strengths of the PACE model&#x3a; support is built around the individual, with the goal of helping participants achieve the highest possible quality of life while maintaining their independence.&#xd;&#xa;&#xd;&#xa;A Meaningful Measure of Success&#xd;&#xa;Survey results like these are especially meaningful because they come directly from the people we serve.&#xd;&#xa;&#xd;&#xa;When more than half of survey respondents tell us they are living independently, it reinforces the importance of providing coordinated, person-centered care that supports the whole individual.&#xd;&#xa;&#xd;&#xa;At Colorado PACE, we are proud to be part of a model of care that helps older adults remain active, engaged and connected to their communities.&#xd;&#xa;&#xd;&#xa;Because independence is more than a goal.     <title>From Colorado PACE to the Church Pew&#x3a; Leon&#x27;s Journey Back to Independence</title>

    <description>Leon Bryant is proud to be from DC. No, not that DC. When Leon says &#x22;DC,&#x22; he&#x27;s talking about Denver, Colorado.&#xd;&#xa;&#xd;&#xa;A lifelong Denverite, Leon has always been someone who enjoys staying busy. After graduating from high school, he served his country in the U.S. Marine Corps before building a successful career in food service and public transportation. Over the years, he worked as a chef, bus driver, dispatcher, and eventually a supervisor for Denver&#x27;s transit system.&#xd;&#xa;&#xd;&#xa;When Leon retired in 2020, he looked forward to enjoying an active retirement. Instead, a series of serious health challenges changed everything.&#xd;&#xa;&#xd;&#xa;Complications with his legs led to multiple surgeries, leaving him unable to walk without assistance. Everyday activities became difficult, but one loss weighed especially heavily on him.&#xd;&#xa;&#xd;&#xa;For years, Leon had to be wheeled into church.&#xd;&#xa;&#xd;&#xa;When Leon enrolled in Colorado PACE earlier this year, his interdisciplinary care team took the time to understand what mattered most to him. One of his biggest goals wasn&#x27;t simply to walk farther&#x26;&#x23;8212&#x3b;it was to walk into church on his own.&#xd;&#xa;&#xd;&#xa;The team got to work.&#xd;&#xa;&#xd;&#xa;They found Leon a pair of supportive shoes that fit comfortably&#x26;&#x23;8212&#x3b;a milestone in itself.&#xd;&#xa;&#xd;&#xa;&#x22;It was the first pair of shoes I was able to wear for six years&#x21;&#x22; he said.&#xd;&#xa;&#xd;&#xa;With encouragement from Colorado PACE&#x27;s rehabilitation team, Leon began practicing his walking at the Colorado PACE Center. Step by step, his strength and confidence returned.&#xd;&#xa;&#xd;&#xa;Then came the moment he had been working toward.&#xd;&#xa;&#xd;&#xa;One Sunday morning, Leon walked into church under his own power and sat in the seat of his choice.&#xd;&#xa;&#xd;&#xa;It may have seemed like a simple walk to others, but for Leon, it represented independence, determination, and the joy of reclaiming an important part of his life.&#xd;&#xa;&#xd;&#xa;Today, Leon is grateful for the Colorado PACE team that stood beside him every step of the way.&#xd;&#xa;&#xd;&#xa;&#x22;The staff is really friendly, and they really are concerned about how things are going for you,&#x22; he said. &#x22;They take care of you.&#x22;&#xd;&#xa;&#xd;&#xa;At Colorado PACE, care is never just about treating a medical condition. It&#x27;s about listening to what matters most to each participant and helping them achieve the goals that bring purpose, confidence, and joy to their lives. For Leon, that goal was walking into church on his own. Thanks to a dedicated team, compassionate care, and Leon&#x27;s determination, that goal became a reality&#x26;&#x23;8212&#x3b;and every step forward is a reminder of what&#x27;s possible when people are empowered to live life on their own terms.&#xd;&#xa;&#x26;&#x23;160&#x3b;</description>

    <description>To describe Josie Lobato&#x27;s life as active would be a serious understatement. Over the years, she has raised eight children, worked at the Colorado Historical Society, written books, and created stunning colcha embroidery&#x26;&#x23;8212&#x3b;traditional Southwestern folk art rooted in the Hispanic and Indigenous cultures of northern New Mexico and southern Colorado. She was even awarded a National Heritage Fellowship medal from the National Endowment for the Arts in 2019&#x21;&#xd;&#xa;&#xd;&#xa;Her work, like her life, is rich with color, history, and purpose.&#xd;&#xa;&#xd;&#xa;As Josie approaches her 90th birthday, age and health challenges have made it harder for her to live at the pace she once did. &#x22;It&#x27;s important to keep yourself entertained,&#x22; she says, reflecting on the frustration of slowing down.&#xd;&#xa;&#xd;&#xa;Through Colorado PACE, Josie has found a way to remain active, creative, and connected. She visits the Colorado PACE Center three times a week, and those days are often the highlight of her schedule. &#x22;I would never have been happy in a nursing home,&#x22; she shares. &#x22;This has been a blessing to me.&#x22;&#xd;&#xa;&#xd;&#xa;Perhaps most meaningful of all, Colorado PACE has allowed Josie to remain independent in her own home&#x26;&#x23;8212&#x3b;where she can continue caring for her beloved cat, Mika.     <description>Barb Curtis may be a participant at Colorado PACE, but she&#x27;s also become one of our most enthusiastic ambassadors.&#xd;&#xa;&#xd;&#xa;Barb began receiving services from Colorado PACE in October after growing frustrated with the support she was receiving from a previous provider. Since then, her experience has been transformative. &#x22;Colorado PACE has been a godsend,&#x22; she says. &#x22;I just can&#x27;t say enough good things about it. Every person here cares about you, listens to you, and gives you the opportunity to make your own choices.&#x22;&#xd;&#xa;&#xd;&#xa;Many of Barb&#x27;s choices at Colorado PACE center around her love of the creative arts. She is especially skilled at diamond art, a detailed craft that uses tiny resin &#x22;diamonds&#x22; placed on a sticky canvas to create colorful, shimmering designs. Several of Barb&#x27;s pieces are proudly displayed throughout the Colorado PACE Center, including a majestic eagle she created with great care and patience.&#xd;&#xa;&#xd;&#xa;Barb credits Colorado PACE with improving her life in meaningful ways. &#x22;I am not the same person I was before,&#x22; she shares. &#x22;Colorado PACE has helped me come out of my shell.&#x22; Anyone who knows Barb can attest to that&#x26;&#x23;8212&#x3b;she&#x27;s quick to share her positive experience with friends, neighbors, and even strangers, often encouraging others to explore Colorado PACE for themselves.&#xd;&#xa;&#xd;&#xa;Barb attends the Colorado PACE Center five days a week and says simply, &#x22;When I&#x27;m not here, I miss it.&#x22; We miss you when you&#x27;re not here too, Barb&#x26;&#x23;8212&#x3b;and we&#x27;re grateful to have you as part of the Colorado PACE family and as a passionate advocate for the work we do.&#xd;&#xa;&#x26;&#x23;160&#x3b;</description>
