At Colorado PACE, care extends far beyond medical appointments and clinical services. Supporting emotional well-being, social connection, and quality of life is an essential part of helping older adults remain healthy, independent, and engaged in their communities.

That commitment is reflected in participant feedback: 92% of Colorado PACE participants report satisfaction with the emotional and social support they receive.
This level of satisfaction highlights the importance of whole-person care — an approach that recognizes health is shaped not only by physical conditions, but also by emotional wellness, stable housing, social engagement, and access to supportive resources.

Colorado PACE works closely with participants to understand their individual needs and connect them with services and support systems that can improve daily life. Whether helping address social isolation, providing emotional encouragement, coordinating supportive services, or assisting with housing-related concerns, the care team focuses on treating each participant with dignity, compassion, and respect.

Strong emotional and social support can have a meaningful impact on overall well-being. Older adults who feel connected and supported often experience improved mental health, greater confidence, and a stronger sense of independence. That is why Colorado PACE prioritizes building relationships and creating an environment where participants feel heard and valued.

The PACE model is designed to help older adults remain safely in their homes and communities for as long as possible while receiving comprehensive care tailored to their unique needs. By integrating medical care with emotional and social support services, Colorado PACE helps participants live fuller, healthier lives.

As Colorado's aging population continues to grow, whole-person care remains more important than ever. Colorado PACE is proud to support participants not only as patients, but as individuals with unique stories, experiences, and goals.

Providing high-quality care doesn't stop when a participant leaves a healthcare center. At Colorado PACE, care continues in the places where participants live their daily lives.

Results from the program's first Annual Participant Satisfaction Survey show that 85% of participants reported being satisfied with their home care visits.

Home-based care plays an important role in helping older adults remain safe, independent, and supported while living in their own communities. Through the PACE model, interdisciplinary care teams coordinate services that may include nursing visits, therapy, personal care, and other support delivered directly in participants' homes.

These services ensure that participants receive consistent, high-quality care regardless of location. The same commitment to safety, compassion, and clinical excellence found in Colorado PACE centers extends to home visits throughout the community.

Home care visits also help care teams better understand each participant's environment and daily challenges. By seeing firsthand how participants live, care providers can identify potential risks, recommend safety improvements, and tailor care plans to better support independence.

The strong satisfaction rating in the survey reflects Colorado PACE's commitment to meeting participants where they are—both geographically and personally.

As Colorado PACE continues to expand its services, participant feedback remains a critical tool for maintaining high standards and ensuring care is delivered in ways that truly support aging in place.

This collaborative model allows participants to receive the care they need while maintaining independence and staying connected to their communities.

As Colorado's population continues to age, programs like PACE are becoming increasingly important in providing high-quality healthcare for seniors, reducing unnecessary hospitalizations, and supporting family caregivers.

The results of the first statewide participant satisfaction survey demonstrate the program's impact and reinforce its role as a trusted option for integrated senior care and aging-in-place services in Colorado.

Colorado PACE Survey Shows 90% of Participants Understand and Are Satisfied with Their Plan of Care

At Colorado PACE, providing care that reflects each participant's unique needs and goals is central to the organization's mission. Results from the program's first Annual Participant Satisfaction Survey show that this approach is making a meaningful difference.

According to the survey, 90% of Colorado PACE participants reported that they understand and are satisfied with their plan of care.

A clear, personalized plan of care is a cornerstone of the PACE model. Participants work closely with an interdisciplinary care team—including physicians, nurses, social workers, therapists, and other specialists—to develop a plan tailored to their health needs, preferences, and daily lives.

The strong survey results demonstrate the effectiveness of this collaborative approach. When participants understand their care plans and feel confident in them, they are better able to engage in their care and maintain independence.

At Colorado PACE, care planning is not a one-time conversation. Plans are continually reviewed and adjusted as participants' needs change, ensuring care remains responsive and individualized.

These early survey results reinforce Colorado PACE's commitment to meeting participants where they are and partnering with them to support their health, well-being, and quality of life.

As the program continues to grow, participant feedback will remain a critical part of shaping and improving the care experience.

Colorado PACE Participant Spotlight: Josie Lobato

To describe Josie Lobato's life as active would be a serious understatement. Over the years, she has raised eight children, worked at the Colorado Historical Society, written books, and created stunning colcha embroidery—traditional Southwestern folk art rooted in the Hispanic and Indigenous cultures of northern New Mexico and southern Colorado. She was even awarded a National Heritage Fellowship medal from the National Endowment for the Arts in 2019!

Her work, like her life, is rich with color, history, and purpose.

As Josie approaches her 90th birthday, age and health challenges have made it harder for her to live at the pace she once did. "It's important to keep yourself entertained," she says, reflecting on the frustration of slowing down.

Through Colorado PACE, Josie has found a way to remain active, creative, and connected. She visits the Colorado PACE Center three times a week, and those days are often the highlight of her schedule. "I would never have been happy in a nursing home," she shares. "This has been a blessing to me."

Perhaps most meaningful of all, Colorado PACE has allowed Josie to remain independent in her own home—where she can continue caring for her beloved cat, Mika. As Josie puts it best, "You really can't find a better place for services."

We are grateful to Josie for trusting Colorado PACE with her care and honored to share her inspiring story.

Colorado PACE Participant Spotlight: Barb Curtis

Barb Curtis may be a participant at Colorado PACE, but she's also become one of our most enthusiastic ambassadors.

Barb began receiving services from Colorado PACE in October after growing frustrated with the support she was receiving from a previous provider. Since then, her experience has been transformative. "Colorado PACE has been a godsend," she says. "I just can't say enough good things about it. Every person here cares about you, listens to you, and gives you the opportunity to make your own choices."

Many of Barb's choices at Colorado PACE center around her love of the creative arts. She is especially skilled at diamond art, a detailed craft that uses tiny resin "diamonds" placed on a sticky canvas to create colorful, shimmering designs. Several of Barb's pieces are proudly displayed throughout the Colorado PACE Center, including a majestic eagle she created with great care and patience.

Barb credits Colorado PACE with improving her life in meaningful ways. "I am not the same person I was before," she shares. "Colorado PACE has helped me come out of my shell." Anyone who knows Barb can attest to that—she's quick to share her positive experience with friends, neighbors, and even strangers, often encouraging others to explore Colorado PACE for themselves.

Barb attends the Colorado PACE Center five days a week and says simply, "When I'm not here, I miss it." We miss you when you're not here too, Barb—and we're grateful to have you as part of the Colorado PACE family and as a passionate advocate for the work we do.

Colorado PACE Survey Shows 90% of Participants Feel Encouraged to Be Part of Their Care Decisions

Colorado PACE recently completed its first Annual Participant Satisfaction Survey, and the results reveal strong feedback from the individuals served by the program.

One of the most encouraging findings: 90% of participants reported that they feel encouraged to be involved in their own care decisions.

This feedback reflects a core principle of the Colorado PACE model—ensuring that participants remain active partners in their care planning.

Empowering Participants in Their Care
For many older adults and individuals with complex health needs, maintaining independence and having a voice in care decisions is incredibly important. Colorado PACE is designed to support this by building care plans with participants, not just for them.

Participants work closely with an interdisciplinary care team that listens to their preferences, goals, and concerns. This collaborative approach helps ensure that each care plan reflects the individual's needs, lifestyle, and priorities.

The survey results demonstrate that participants are experiencing this approach firsthand.

Why Participant Feedback Matters
Participant feedback is an important part of improving care. Surveys like this help programs understand what is working well and where opportunities exist to make services even better.

The overwhelmingly positive response from participants reinforces Colorado PACE's commitment to:

	Person-centered care
	Participant engagement
	Collaborative decision-making
Respect for individual preferences and goals

Continuing to Build on Positive Results
While the survey results are encouraging, Colorado PACE views this feedback as part of an ongoing conversation with the people it serves. Listening to participant experiences helps guide improvements and ensures the program continues to meet the needs of the community.

Encouraging individuals to take an active role in their care helps build trust, confidence, and stronger health outcomes.

For Colorado PACE, the message from participants is clear: having a voice in care decisions matters—and it's making a difference.

Colorado PACE Participant Satisfaction Survey Shows Strong Support for Independent Living

Colorado PACE Survey Results Highlight Participant Satisfaction and Independence

Colorado PACE recently received the results from its first Annual Participant Satisfaction Survey, offering valuable insight into how the program is supporting older adults across the region.

The feedback was overwhelmingly positive, with participants sharing experiences that demonstrate the impact of coordinated care, personalized support, and comprehensive services designed to help older adults remain safe, healthy, and independent.

One key finding stood out: more than 50% of participants who responded to the survey reported that they are living independently.

Supporting Independence Through the PACE Model of Care
The Program of All-Inclusive Care for the Elderly (PACE) is designed to help older adults maintain independence while receiving the medical care and supportive services they need.

Colorado PACE provides coordinated healthcare, social services, rehabilitation support, and other resources that allow participants to continue living in their homes and communities whenever possible.

The survey results demonstrate that this model is working. By focusing on whole-person care and individualized support, Colorado PACE helps participants maintain autonomy and quality of life.

Listening to Participants and Improving Care
Participant feedback is an important part of ensuring that programs continue to meet the needs of the people they serve. The Annual Participant Satisfaction Survey helps Colorado PACE better understand participants' experiences and identify opportunities to enhance services.

These early results reflect both the strength of the PACE model and the dedication of the care teams who support participants every day.

Helping Older Adults Age in Place
For many older adults, the ability to remain in their own home is closely connected to dignity, comfort, and overall well-being. Programs like Colorado PACE play an important role in making aging in place possible.

By providing coordinated care and supportive services, Colorado PACE empowers participants to maintain independence while receiving the medical and social support they need.
