Colorado PACE Survey Shows 90% of Participants Understand and Are Satisfied with Their Plan of Care
First Annual Participant Satisfaction Survey highlights the power of person-centered care
At Colorado PACE, providing care that reflects each participant’s unique needs and goals is central to the organization’s mission. Results from the program’s first Annual Participant Satisfaction Survey show that this approach is making a meaningful difference.
According to the survey, 90% of Colorado PACE participants reported that they understand and are satisfied with their plan of care.
A clear, personalized plan of care is a cornerstone of the PACE model. Participants work closely with an interdisciplinary care team—including physicians, nurses, social workers, therapists, and other specialists—to develop a plan tailored to their health needs, preferences, and daily lives.
The strong survey results demonstrate the effectiveness of this collaborative approach. When participants understand their care plans and feel confident in them, they are better able to engage in their care and maintain independence.
At Colorado PACE, care planning is not a one-time conversation. Plans are continually reviewed and adjusted as participants’ needs change, ensuring care remains responsive and individualized.
