Day Center

Day Center

The Colorado PACE Day Center is designed to offer the care and support services each participant needs in one convenient location. Participants visiting the Day Center experience recreational activities, eat healthy meals, as well as develop new friendships and companionship. In addition, trained and specialized clinical staff provide nutritional counseling, physical therapy, occupational therapy, exercise, and more. Our goal is to offer comprehensive care and activities that help seniors stay healthy, active, and develop a strong sense of community.

Participants visit medical professionals on-site at the Colorado PACE clinic for their general health care needs. Referrals are made to appropriate health specialty providers such as oncologists, podiatrists, cardiologists, and a variety of other specialty care providers for additional care as defined in each participant’s individualized care plan.

In-Home Care

In-Home Care

Personal care assistance in the home is an essential part of maintaining independence. Colorado PACE may provide skilled home health and/or home care for participants who meet the need for these services as determined by their individualized care plan developed by the Colorado PACE Interdisciplinary Team.

Colorado PACE provides transportation from home to the Colorado PACE Day Center and to off-site medical specialty appointments as identified in each participant’s individualized care plan.

Social Work

Social workers partner with each Colorado PACE participant and their families/loved ones. Social workers provide sensitive and caring support that addresses the emotional, family, and social needs of all involved.

Rehabilitative Therapy Services

The Colorado PACE Interdisciplinary Team will assess each participant’s abilities and activity goals. They will prescribe physical, occupational, speech, and/or rehabilitative therapies as part of each participant’s individualized plan of care.

The Colorado PACE behavioral health team works closely with the Interdisciplinary Team and other health care providers to address a variety of concerns including depression, anxiety, sleep, substance use, etc.

Colorado PACE Day Center recreation specialists focus on developing and leading fun and engaging activities such as musical entertainment, bingo and other games, art projects and group exercise. Participants can also enjoy breakfast and lunch at the Colorado PACE Day Center.

If I Belong to a Medicare HMO, Can I Also Enroll in Colorado PACE For Long-Term Care Services?2024-06-30T22:22:02-06:00

The Colorado PACE program is both Medicare providers and long-term care providers. By enrolling in the Colorado PACE program, you will no longer be enrolled in a traditional Medicare/HMO plan if you have one.

Can I Keep My Own Doctor?2024-09-02T10:26:10-06:00

Colorado PACE participants agree to receive all health care services defined in a participant’s individualized care plan developed by the Colorado PACE program’s Interdisciplinary Team (IDT). Colorado PACE IDT members include the primary care provider, registered nurse, social worker, physical, occupational, and recreational therapists, dietitian, home care coordinator, personal care attendant, transportation driver, and center director.

If you have a specialist that is part of the Colorado PACE network of contracted providers, you may be able to keep your current specialty care physician. When a participant enrolls in a Colorado PACE health plan, she/he is assigned a primary care physician (PCP) who is a member of the Colorado PACE Day Center’s Interdisciplinary Team (IDT) skilled in treating the healthcare needs of seniors.

Colorado PACE participants may be fully and personally liable for the costs of unauthorized or out of Colorado PACE program agreement services.

How Do Program Participants Get to the Colorado PACE Day Center?2024-09-02T10:31:50-06:00

Colorado PACE programs provide transportation to the Day Center. Transportation is a key part of the Colorado PACE benefit. Transportation is not only provided between the home and the Day Center, but also to appointments with specialists and other activities that are included in each Colorado PACE participant’s individualized care plan.

At the Colorado PACE Day Center, a participant may see a care provider in the clinic, have lunch, enjoy social activities, and receive therapies before being taken back to their home and the end of the day.

Can I Just Use a Specific Service Such as Home Care, Meals, Transportation or The Day Center?2024-09-02T11:28:31-06:00

No, that is not recommended. Focusing on a single aspect of care does not provide seniors in the Colorado PACE program with the best chance of remaining independent in the community. For example, a participant may need rehabilitation after a stroke, which could include a special diet or medications for hypertension, diabetes, and other chronic problems. A participant may also feel lonely and isolated. The Colorado PACE program can address these problems through a team of experienced healthcare professionals and a treatment plan that focuses on a participant’s complete health care picture.

Do PACE Participants Attend the Day Center Every Day?2024-09-02T10:32:29-06:00

Not usually. On average, Colorado PACE participants attend the Day Center two to three times a week. Colorado PACE Day Center attendance is based on the needs defined in a participant’s individualized care plan and can range from once a week to several days a week, as needed.
