What is PACE?

Colorado PACE is a non-profit senior healthcare organization that provides nursing home eligible seniors with the services needed to be able to live independently in their own homes and communities for as long as it is safely possible. Colorado PACE is a Health Plan with a Medicare Contract.

We are committed to helping you:

The Colorado PACE Interdisciplinary Team (IDT)

Colorado PACE offers a comprehensive continuum of services customized for each individual participant.
The IDT team at the Colorado PACE Day Center coordinates all your health services.

Benefits of PACE

  • Participating in Colorado PACE simplifies access to healthcare and prevents the fragmented approach to care delivery.

  • Services are coordinated 24 hours a day.

  • The Colorado PACE Interdisciplinary Team (IDT) will develop a personalized care plan with you. Along with you, your family and/or caregiver, Colorado PACE will monitor your physical and mental health status and provide timely interventions as needed to maximize your health and wellness.

  • Your primary care physician’s (PCP) office is located at the Colorado PACE Day Center clinic where most of your healthcare services will be provided.

  • There are no deductibles or co-payments for any drug, service, or care that is approved by the IDT.

  • Transportation is arranged from home to the Colorado PACE Day Center for health care services approved by IDT and included in your personalized care plan.

  • Ongoing support for you, your caregiver and/or family is provided.

  • You will have a team of health care providers whose mission is to meet your needs through physical, emotional, and social supportive care that preserves your dignity and provides you with greater independence.
